District Driver List
To be able to drive student on Field Trips and for Sports all drivers must be registered with the district.
Submit the following forms the District Office:
$300,000 each occurrence
Property Damage $25,000 each occurrence
Or
Combined Single Limit $300,000 each occurrence
Contact the District Office for printed copies of these forms.
Submit the following forms the District Office:
- Completed and signed Authorization for Release of Driver Record
- Completed and signed Driver/Chaperone Contract
- Completed and signed Application to Provide Student Transportation, and
- Proof of Insurance showing minimum limits of coverage:
$300,000 each occurrence
Property Damage $25,000 each occurrence
Or
Combined Single Limit $300,000 each occurrence
